Connected Inc NDIS is in a unique position to provide NDIS services that support individuals and their families experiencing Psychosocial (the new term for mental illness and/or dual diagnosis) and Acquired Brain Injury Disabilities
Services We Provide

Assisting you to complete personal tasks of daily life to enable you to live as autonomously as possible.

Assisting you to participate actively in community, social and civic activities; includes supporting you during these activities and developing your ability to partake in these activities.

Short and long-term supports that focus on strengthening your ability to coordinate your supports, and to assist you to live at home and participate in your community. These supports consist of:
• Support Connection
This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.
• Coordination of Supports
This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
• Assistance with Accommodation and Tenancy Obligations
• Life Transition Planning
This includes mentoring, peer support and individual skill development.
• Assistance with Decision Making, Daily Planning, Budgeting
• Assistance with Travel/Transport Arrangements
Our Disability Support Workers are specifically trained by our Disability Services Manager in mental health and illness, acquired brain injuries and an eclectic range of therapeutic communication skills and best practice strategies to support and empower everyone we work for.
NDIS Participants, Families and External Providers can be assured that here at Connected Inc NDIS, we provide respectful, genuine professional person-centred support to enable the individual to enjoy a life less focused on disability but rather on the unique wonderfulness of their whole person thereby creating a flourishing rewarding life.
About Our Disability Services Manager
Leesa, our DSM, driven by a dream and powered by a lifetime of experience, devotes herself to her work. Her dream that each and every individual receives not only high quality support, but the ability to thrive. Her dream that each and every individual no longer feels like “Just a Number”.
Her decades of experience enables her to work with a participant closely to devise, implement and review clear and empowering support plans. Coupling this with our friendly and caring Disability Support Workers, Leesa is able to receive the best payment- a participant’s success!
